    The presence of strongyloidiasis and associated risk factors in patients undergoing treatment at the National Cancer Institute, Maharagama, Sri Lanka

    INTRODUCTION: Strongyloides stercoralis can cause severe disease in the immunocompromised. Without a proper gold-standard diagnostic technique, strongyloidiasis is scarcely studied both globally and locally. OBJECTIVES: We aimed to estimate the prevalence of strongyloidiasis among immunocompromised adult patients and to identify risk factors. METHODS: This study was carried out between February to October 2022. A faecal sample and 2 ml of venous blood were collected from consented patients. Direct faecal smear, agar plate, Harada-Mori and Charcoal cultures were performed on the faecal samples. Qualitative Polymerase Chain Reaction (PCR) was performed on selected faecal samples using S. stercoralis targeting ITS1 region. Strongyloides IgG ELISA was carried out on the serum samples using DRG Strongyloides IgG ELISA kit. RESULTS: Collectively, 144 patients (males = 68, females = 76) provided blood/faecal sample or both. Relevant to strongyloidiasis-associated symptoms, some patients had diarrhoea (n=12) and eosinophilia (n=11). Some of them (n=74) had occupational or recreational exposure to soil as potential risk factors. Overall, 24 patients were positive for strongyloidiasis from one or more diagnostic method (5 PCR and 19 ELISA). There were zero culture or direct smear positives. There was no significant association between disease positivity with either of the clinical features or risk factors. CONCLUSION: The prevalence of strongyloidiasis in patients with malignancies was 16.66%. Strongyloidiasis is existent in the immunocompromised in Sri Lanka even in the absence of suggestive clinical features or regular exposure to risk factors. Screening immunocompromised patients with sensitive techniques such as PCR for timely diagnosis and treatment is recommended.

    A preliminary survey for filarial parasites among dogs and cats in mahawewa, puttalam and their vector identification

    PURPOSE This study investigated the prevalence of zoonotic filarial parasites within the canine and feline population in Mahawewa, Puttalam, and their vectors based on a brugian filariasis positive human case reported to the Anti-filariasis Campaign in January 2021.METHODS & MATERIALS All reachable dogs and cats, both stray and domestic, within a 500m radius of the index human case of brugian filariasis were screened for microfilaria using Giemsa stained thick blood smears prepared from capillary blood, obtained from an ear-lobe prick. Mosquito collection was done using a dog-baited trap, two window traps and a B.G. Sentinel trap from the same study site and identified using morphological keys. The head and the thorax regions of randomly selected mosquito specimens were dissected for morphological identification of larval filaria parasites via microscopy. RESULTS A total of nine dogs and three cats were surveyed, of which seven dogs and one cat had filarial infections. All the infected animals harbored B.malayi microfilariae, while four dogs and one cat were co-infected with Dirofilaria repens and two dogs with an unidentified species. A total of 119 mosquitoes were caught and identified by taxonomic keys using a dog-baited trap, two window traps and a B.G. Sentinel trap from the study site. Dissection of heads and thoraces of randomly selected 12 Mansonia annulifera, 18 Mansonia indiana 20 Mansonia uniformis and 8 Culex spp. revealed filarial larvae in M.annulifera (n=4, 33.33%), M. indiana (n=14, 77.78%), M. uniformis (n=10, 50.00%) and Culex spp.(n=5, 62.5%) via microscopy. CONCLUSION M.indiana was incriminated as a potential vector of filarial parasites for the first time in Sri Lanka. Preliminary evidence generated indicate a high prevalence of B.malayi and D.repens among dogs and cats in Mahawewa, Puttalam with an abundance of mosquito vectors mostly of Mansonia and Culex spp. This warrants further studies with a larger sample size and molecular identification of the filarial larvae within mosquito and animal samples, especially as re-emergence of brugian filariasis in humans is being reported after four decades of quiescence, and a zoonotic brugian parasite has been detected over the recent past, raising a concern from a one health perspective.

    The validation of the Sinhala version of the Kessler psychological distress scale (K10) to screen for psychiatric morbidity

    BACKGROUND: The Kessler psychological distress scale (K10), used in epidemiological surveys, measures psychological distress. High scores in community surveys are associated with anxiety and affective disorders, and to a lesser extent, with other psychiatric disorders. OBJECTIVE: To validate the Sinhala translations of the long (K10) and short (K.6) versions of the Kessler psychological distress scale. DESIGN, SETTING AND METHODS: The English version of K10 was translated into Sinhala. Content and face validity was assessed by experts. The scales were pre-tested and modified accordingly. The Sinhala versions of K6 and K10, and the Structured Clinical Interview Schedule were administered to 20 adults with major psychiatric illnesses diagnosed by two clinicians independently, and to a random sample of 25 apparently normal people from the community. SPSS (Version 11) was used for the analysis. RESULTS: The ROC curve for the K10 contained 96.1% of the area under the curve of 0.961 (95% CI 90.4%-100%). A cut off score of 22 for the K10 yielded a sensitivity of 93.8% and a specificity of 82.6%. The ROC curve for the K6 contained 90.1% (95% CI 80.5% - 99.7%) of the area under the curve. For the K6, a cut off score of 13 gave a sensitivity of 88.2% and a specificity of 72%, The total number of days that the patient could not attend to regular work and responsibilities was significantly correlated with both the K10 (p=0.041) andK6 (p=0.023). CONCLUSION: The Sinhala version of the K10 and K6 questionnaires can be used to screen for psychological distress.

    The Ragama Health Study: the methodology of the prospective cohort study for the establishment of diagnostic criteria for metabolic syndrome in Sri Lankans

    BACKGROUND: Sri Lanka is in the midst of the epidemiologic transition with non-communicable diseases being a leading cause of death and hospitalization. This pilot study is a part of an international study conducted by the International Medical Centre of Japan (IMCJ) in collaboration with the Faculty of Medicine, University ofKelaniya. OBJECTIVE; To determine the prevalence of major metabolic disorders and to establish diagnostic criteria for metabolic syndrome in the Sri Lankan population as a pilot study. DESIGN, SETTING AND METHODS: A random sample of 3500 adults 35-64 years was selected from the electoral register. Houses of selected subjects were visited and the selected subject invited to participate in the study. Subjects were instructed to fast for 12 hours and refrain from smoking and consumption of alcohol overnight prior to presenting at the Family Medicine clinic of the Faculty of Medicine, University of Kelaniya. At the clinic, subjects were assigned an unique identification number and a detailed history taken and investigations carried out. Heights, weights, blood pressure and waist and hip circumference were measured using standard techniques. Subjects underwent an ultrasound scan of the liver and a sample of blood was obtained for full. blood count, blood picture, lipid profile, serurn insulin, serum alanine transferase, fasting blood sugar and for genetic analysis. Samples of blood for genetic analysis have been stored at -30° C until further analysis. In addition, subjects were administered a food frequency questionnaire and an assessment of daily physical activities recorded. All subjects with abnormal results of investigations are being followed up.

    Filarial dance sign (FDS) in patients with lymphatic illariasis

    BACKGROUND: Lymphatic filariasis causes acute lymphangitis, epididyrno-orchitis, hydrocoele, lymphoedema and nocturnal cough. Diagnostic tests based on circulating filarial antigens (CFA) and filarial antibodies (FAT) have limitations in confirming symptomatic filariasis. Filaria dance sign (FDS) demonstrated using soft tissue ultrasonography permits identification of live adult filarial worms in-situ. Objectives: FDS, CFA and FAT status in patients with clinical features suggestive of lymphatic filariasis, DESIGN, SETTING AND METHODS: Adult males with symptoms suggestive of filarial infection were subjected to scrotal scans using a Toshiba 7.5MHz soft tissue transducer to elicit the FDS. All subjects were screened for CFA and FAT by NOW® Filariasis (Binax Inc. USA) and On-Site Filariasis IgG/IgM Rapid Test (Biotech. Inc. USA) respectively. RESULTS: Forty eight males, mean age 48.5 yrs (SD: 15.2), presenting with lymphoedema of lower limbs (LL, n=20), lower limb cellulitis with lymphangitis (LCL, n=7), hydrocoele (H, n=7). acute epididymo-orchitis (AEO, n=3), hydrocoele with lower limb lymphoedema (HLL, n=2) and nocturnal-cough (NC, n=9) were studied. FDS was demonstrated in 38(79%); 7 patients with H, 16 with LL, 5 with LCL, AEO 1 and 9 with NC. Six of 41 (14.6%) patients tested for filarial antibodies were positive for filaria-specific IgG; 2 of them were also positive for filaria-specific IgM. Two of the six IgG positives were negative for FDS. The 4 IgG and FDS positives had LCL (n=2), H (n=l) and AEO (n=l). All were CFA negative. CONCLUSIONS: Although time consuming, demonstration of FDS by soft tissue ultrasonography can be useful in confirming symptomatic filariasis compared to FAT and CFA.
