Item Acceptability of a web-based character strengths module for early adolescent school children(Postgraduate Institute of Medicine University of Colombo, 2023) Chandradasa, M.; Williams, S.; Sumanasiri, S.; Selliah, S.; Gunarathne, P.; Paric, A.; Ravindran, N.; Kuruppuarachchi, K.A.L.A; Ravindran, A.BACKGROUND: Frequent socio-political conflicts in the past decades have adversely affected the well-being of youth in Sri Lanka. There is a need to support youth development, mitigate past trauma, and promote social harmony. School-based interventions to improve character development, emotional regulation, and tolerance have good evidence and may benefit adolescents in Sri Lanka. Our objective was to develop a novel character strength program in Sinhala to promote Leadership, Empathy, Altruism, Personal Growth, and Social Responsibility (LEAPS). This program consists of ten web-based modules and is facilitated by teachers. This initial evaluation focuses on the first module and its acceptability among students and teachers. METHOD: Program modules were developed by a child and adolescent psychiatrist with the support of mental health and educational experts. The first module was piloted among grade 8 students (12-14 years) and teachers of two schools in the Gampaha District. It was designed to be interactive and contains various cultural activities and questions. On completion, participants gave feedback online via Likert scales. Ethical approval for the study was obtained from the Faculty of Medicine, University of Kelaniya. RESULTS: A total of 115 students and 66 teachers participated. Overall, more than 95% of students and teachers found the module appropriate and suitable. The students had more favourable ratings for the module compared to the teachers on aspects of understanding the content (p<0.001), ability to complete by self (p<0.001), suitability to age (p<0.001), shorter time for completion (p<0.001) and suitability of the design (p<0.001). Both groups suggested that the pictures and activities be increased. Liebowitz Social Anxiety Scale(LSAS-SR) is one of the most widely used scales to screen for social phobia and it evaluates fear and avoidance symptoms associated with the above disorder.Item Perceived functional impairment and spirituality/religiosity as predictors of depression in a Sri Lankan spinal cord injury patient population(Houndmills, Basingstoke, Hampshire, UK : Stockton Press, 2016) Xue, S.; Arya, S.; Embuldeniya, A.; Narammalage, H.; da Silva, T.; Williams, S.; Ravindran, A.STUDY DESIGN:Cross-sectional, questionnaire-based study.OBJECTIVES:To test the hypothesis that self-perceived functional impairment and religiosity/spirituality (S/R) predict depression among traumatic spinal cord injury (SCI) patients in Sri Lanka. SETTING: Ragama Rheumatology and Rehabilitation Hospital, Ragama, Sri Lanka. METHODS: The Spinal Cord Independence Measure, Benefit Through Spirituality/Religiosity Scale, Sheehan Disability Inventory and Beck Depression Inventory-II (BDI-II) were administered to 61 consenting in-patients with traumatic SCI between June and July 2014. A linear regression model on BDI-II score was developed to examine the impact of self-perceived functional impairment and S/R activities on psychiatric outcomes in context of various sociodemographic variables.RESULTS: Psychiatric consequences of SCI were reflected in a 41% prevalence of depression. Thirty-six percent (R2=0.36) of the variance in BDI-II scores (F(5, 55)=6.07, P<0.001) was explained by the regression model. Functional impairment (β=0.54, t(55)=4.73, P<0.001) and perceived benefit through S/R activities (β=-0.31, t(55)=-2.55, P<0.05) emerged as the strongest predictors for depression severity.CONCLUSIONS: Perceived functional impairment in work, social and family domains predicted depressive symptomatology among SCI inpatients in Sri Lanka, while perceived benefit through S/R protected against depression. The findings emphasize the need for rehabilitative programming to support patients' S/R activities and mental wellbeing, promoting reintegration into their community roles.Item Randomized controlled trial evaluating the effectiveness of cognitive behaviour group therapy (CBGT) compared to a waitlist control in the treatment of university students with social anxiety disorder(Postgraduate Institute of Medicine University of Colombo, 2021) Hapangama, A.; de Silva, R.; Williams, S.S.; de Zoysa, P.T.; Wickremasinghe, R.R.; Kuruppuarachchi, K.A.L.A.; Ravindran, A.BACKGROUND: Social anxiety disorder (SAD) is a common mental illness that causes significant functional impairment. Individually delivered cognitive behavioural therapy (CBT) is an effective but costly intervention with limited access, and is poorly scalable to meet the need in the population. In this context, cognitive behavioural group therapy (CBGT) is a more viable option. OBJECTIVE: To determine the effectiveness of CBGT as compared to a waitlist control for university students with SAD. METHODS: A single-blind randomized controlled trial consisting of an 8-week treatment of CBGT (N=15) versus a waitlist control (N=15) was conducted in university students meeting Diagnostic and Statistical Manual 5 criteria for SAD. The Liebowitz Social Anxiety Scale –Self Rated Sinhala version (LSAS-SR) was administered at screening, baseline, four weeks and eight weeks of the trial. Manualized and Culturally adopted CBGT was delivered in Sinhala language, to the intervention group by a licensed clinical psychologist. RESULTS: The mean age of the study population was 22 (SD=1.345) and baseline mean LSAS-SR score was 72.33 in the experimental arm and 69.03 in the wait list control. Repeated measures analysis revealed that the mean total score of the LSAS-SR in the CBGT arm was significantly lower (13.262; p<0.001) post-intervention compared to the waitlist group after controlling for age and gender. CONCLUSION: Culturally adapted CBGT in Sinhala for SAD is effective in ameliorating social anxiety symptoms among university students. However, individually delivered cognitive behavioural therapy (CBT) is costly, limited by the number of trained therapists and not scalable to meet the need for such therapy in the population. In this context, cognitive behavioral group therapy (CBGT) is the viable option. Objective was to determine the treatment effectiveness of CBGT as compared to a waitlist control of university students with social phobia. METHOD: A single-blind randomized controlled trial consisting of an 8-week treatment phase of CBGT versus a waitlist control was conducted among university students with social phobia. The Liebowitz Social Anxiety Scale –Self Rated Sinhala version (LSAS-SR) was administered at screening, baseline, four weeks and eight weeks of the trial. A culturally adapted CBGT was delivered in Sinhala to the intervention group by a trained clinical psychologist. RESULTS: Thirty university students (15 in each arm) with a mean age of 22, meeting DSM IV criteria for Social Anxiety Disorder were included. Repeated measure analysis of the total scores of the LSAS-SR in the CBGT arm showed statistically significant (p<0.001) reduction in the total scores (13.3) as compared to the waitlist arm after controlling for age and gender. CONCLUSION: CBGT is superior to waitlist control in university students with social phobia.
